If you’re having an appliance fitted (crown, bridge or denture) your dentist should give you a statement of manufacture - this statement proves your dental device has been made to legal standards and is just for you.  You will receive a statement with each new dental appliance but they are not supplied for refurbished dental appliances. 

Details your statement will include:-

• Your name     
• A description of the dental appliance
• The name and address of the supplier (laboratory)
• Confirmation that the dental appliance was custom made for you
• Confirmation that your dental appliance meets legal standards
